Filing forms and requests

Most forms require information from your government id and tax bill, so to ensure a smooth process, please have these items on hand. Once you’re ready, select the filling method that best suits you.
Forms are available electronically through our e-forms system or can be downloaded in pdf form and printed to be filled out and mailed or emailed to the Assessors’ Office.

Electronic Forms (E-Forms)
To fill out an e-form, please do the following: 1. Click the appropriate eForm link;
2. Enter the requested information;
3. Review all information and make corrections as necessary;
4. Submit.

Paper Forms (PDF)
To fill out a paper form do the following:
1. Download the PDF file (open with Adobe Reader);
2. After opening the form, please choose to print it and fill it by hand or fill out the pdf directly electronically;
3. Mail or email the finished form to:
a. Mail: Quincy Assessors Office, 1305 Hancock Street, 1st Floor, Quincy, MA 02169.
b. Email: email the form to the contact email address related to the topic.
